# Medical Record Summarizer

### Generate Summary

Similar to previous versions of Hachiko, the generate summary tab is the same. Take legnthy PDF records (handwritten or text, or both!) and drag/drop them into the PDF window. Add the patient name and select the summary type and output language. \
\
**Summary Types:**\
1\. <mark style="color:$primary;">**Standard**</mark> - our standard comprehensive summary in bullet form and chronological order\
2\. <mark style="color:$primary;">**Paragraph**</mark> - take the standard summary and imagine it written for you in paragraph / medical record format \
3\. <mark style="color:$primary;">**Condensed**</mark> - a faster model focused on the clinical problem at hand, not as comprehensive, but great for urgent situations and fast information \
4\. <mark style="color:$primary;">**Client**</mark> - written at a level that families can understand medical jargon; written at a 10th-grade level to improve understanding\
5\. <mark style="color:$primary;">**Extended**</mark> - this option is very comprehensive and does not skip any pertinent information; lengthy summary but the most comprehensive

### Custom Instructions

By default, the instructions shown here are a refined prompt on top of our model pipeline that provides a comprehensive output based on our internal testing. If you want to push the model further as a one-off or on every summary, you can edit this to your liking. \

*- "In addition, document any client conversations related to diagnoses"*\
*- "Please add the status of remission at the end of each treatment based on feedback from the clinical record"*&#x20;

#### Persistent Custom Instructions

When you edit the custom instruction in the generate summary tab, it is a one-off experience. If you'd like to set your own custom default instruction:&#x20;

## Summary History

By clicking the summary history tab, you have access to all previous summaries you have produced before. You can search by name or context for quick access. \

At the bottom of each summary, you have several options:\
1\. <mark style="color:$primary;">**Copy to clipboard**</mark> - copy the plain text and paste anywhere on your computer (including in your EMR)\
2\. <mark style="color:$primary;">**Export as PDF**</mark> - create a branded PDF that can be provided to your client or imported into the EMR\
3\. <mark style="color:$primary;">**Regenerate**</mark> - if you're looking for more information or accidentally selected the wrong summary type, just regenerate it!\
4\. <mark style="color:$primary;">**Share**</mark> - now you can share summaries with specific patients and teams you work with, meaning the summary is accessible for everyone and for that patient

#### Find Source References

Hachiko now tells you where it found the information, adding an extra layer of confidence as to how information was extracted and where it came from. Simply click <mark style="color:$primary;">**Find Source References**</mark>, wait a few seconds and you can now interact with the summary and identify key events and where they came from.&#x20;
